How to Apply for Duplicate Voter Id card

  4. Proof of Residence (Any one)
    • Passport, Ration Card, Driving License, Income Tax Return filed or Assessment Order
    • Latest Gas Connection, Telephone, Water, Electricity Bill for that address, either in the name of the applicant or that of an immediate relation like the parents.
    • Postal department’s posts received and delivered in the applicant’s name at the given address.
    • Bank, Post Office, Kisan Pass Book
  5. In case of application for lost or misplaced cards, FIR (First Information Report) registered at the time of loss of the voter card is required

  • Section V need not be filled in if the applicant is the same person seeking transposition. In case the person filling the form is seeking transposition for someone else, then section V can be filled in.

  • A unique application ID will be issued which must be preserved for future reference and tracking the application status.
  • The Electoral Officer will verify your application and issue a duplicate card.

Correction/updating Using form 8
